(Advanced Education) If you are applying for Canada-Saskatchewan Integrated Student Loan, this form is completed by your financial institution to confirm any non-redeemable investments and/or penalties associated with liquidating these assets.
  Consent to Release Information Form
(Advanced Education) By completing this form you authorize any individual (other than your school, the Ministry of Advanced Education and/or National Student Loans Service Centre [NSLSC]) to call either the Ministry of Advanced Education and/or NSLSC, on your behalf regarding your student loan account. By naming the individual below, both the Ministry of Advanced Education and/or NSLSC will recognize that the named individual has your permission to call and inquire on your student loan.

  Graduate Retention Program Application
(Advanced Education) The Graduate Retention Program rewards graduates living in Saskatchewan by providing rebate up to $20,000. To be eligible, you have to live and file an income tax return in Saskatchewan. You must apply for the Graduate Retention Program within seven years of graduation. Most Saskatchewan post-secondary institutions apply for the GRP on your behalf. If your post-secondary school doesn't, you must submit an application, along with a copy of your certificate, diploma, or degree.

  Loan Forgiveness for Nurses and Nurse Practitioners Application Form
(Advanced Education) The Saskatchewan Student Loan Forgiveness for Nurses and Nurse Practitioners program encourages nurses and nurse practitioners to work in smaller communities. As a nurse or nurse practitioner, you could receive up to $4,000 per year in Saskatchewan Student Loan forgiveness to a maximum of $20,000 over five years.

  Reduce Income Statement for Married Students
(Advanced Education) The purpose of this form is to determine as close as possible what your gross income will be in the current taxation year. If this gross income will be less than the gross income as shown on Line 150 of last year’s income tax form, contributions can be recalculated using the lower income.
