  Attending Physician's Statement
(Advanced Education) If you are applying for Canada-Saskatchewan Integrated Student Loans and you or your spouse are unable to work due to medical reasons, your contribution or the contribution of your spouse may be waived by submitting the Attending Physicians Statement.

  Canada-Saskatchewan Student Grant - Calendar for Support Services
(Advanced Education) Form is used to keep track of the hours and cost per service used e.g., tutors, scribe, reader, for students who are receiving the Canada Study Grant and Saskatchewan Student Grant for the Accommodation of Students with Permanent Disabilities.
  Change in Information Form
(Advanced Education) This form is used to change information on your Canada-Saskatchewan Integrated Student Loan application.
  Common-Law Relationship Declaration
(Advanced Education) To declare a common-law relationship in reassesment of your Canada-Saskatchewan Integrated Student Loan.
  Confirmation of Investments
(Advanced Education) If you are applying for Canada-Saskatchewan Integrated Student Loan, this form is completed by your financial institution to confirm any non-redeemable investments and/or penalties associated with liquidating these assets.

  Dependent Children Declaration
(Advanced Education) To declare full-time custody of dependent children not listed on the student's health card to be included in the assessment of your Canada-Saskatchewan Integrated Student Loan.
  Dependent Children Over 18 Declaration
(Advanced Education) To declare dependent children that are 18 years of age or older to be included in the assessment of your Canada-Saskatchewan Integrated Student Loan.

  Graduate Retention Program Application
(Advanced Education) The Graduate Retention Program rewards graduates living in Saskatchewan by providing rebate up to $20,000. To be eligible, you have to live and file an income tax return in Saskatchewan. You must apply for the Graduate Retention Program within seven years of graduation. Most Saskatchewan post-secondary institutions apply for the GRP on your behalf. If your post-secondary school doesn't, you must submit an application, along with a copy of your certificate, diploma, or degree.
  Graduate Tax Benefit Programs - Consent to Release Information
(Advanced Education) Consent for a third party to request information pertaining to your graduate tax benefit on your behalf.

  Loan Forgiveness for Nurses and Nurse Practitioners Application Form
(Advanced Education) The Saskatchewan Student Loan Forgiveness for Nurses and Nurse Practitioners program encourages nurses and nurse practitioners to work in smaller communities. As a nurse or nurse practitioner, you could receive up to $4,000 per year in Saskatchewan Student Loan forgiveness to a maximum of $20,000 over five years.

  Parental Separation
(Advanced Education) To declare dependent children receiving student financial assistance upon parental separation.
  Practicum/Internship Form
(Advanced Education) Students receiving Canada-Saskatchewan Integrated Student Loans who will be completing a practicum or internship and require an allowance must complete and submit this form once they know the dates and locations of their practicum/internship.
  Program Information Form
(Advanced Education) The purpose of this form is to provide information on costs and the study period dates of the student’s program. This form does not confirm enrolment.

  Reduced Income Statement
(Advanced Education) If you are a dependent student applying for a Canada-Saskatchewan Integrated Student Loan and your parents' gross income will be less than the gross income as shown on line 150 of last year’s income tax form, the parental contribution can be recalculated using the lower income by submitting this completed form.

  Saskatchewan Designation Policy Manual
(Advanced Education) The Saskatchewan Designation Policy Manual outlines the policies governing the designation of educational institutions for Saskatchewan student financial assistance.
  Saskatchewan Student Grant for Part-time Studies - Confirmation of Enrolment
(Advanced Education) This completed form confirms that the student is enrolled as a part-time student for the purposes of the Saskatchewan Student Grant for Part-time Studies only. The regular process must continue to be followed to confirm enrolment for student loan purposes.
  Separation Declaration
(Advanced Education) To declare dependent children upon parental separation of the student receiving assistance.
  Student Loans Reinstatement Form
(Advanced Education) The Student Loans Reinstatement Form is to be used to reinstate student as full-time within 21 days of discontinuation.
