(Advanced Education) By completing this form you authorize any individual (other than your school, the Ministry of Advanced Education and/or National Student Loans Service Centre [NSLSC]) to call either the Ministry of Advanced Education and/or NSLSC, on your behalf regarding your student loan account. By naming the individual below, both the Ministry of Advanced Education and/or NSLSC will recognize that the named individual has your permission to call and inquire on your student loan.

(Advanced Education) If you are 22 years of age or older and are applying for a Canada-Saskatchewan Integrated Student Loan and Grants for the first time, your credit history for the past three years will be checked before you are awarded financial assistance.

  Reduce Income Statement for Married Students
(Advanced Education) The purpose of this form is to determine as close as possible what your gross income will be in the current taxation year. If this gross income will be less than the gross income as shown on Line 150 of last year’s income tax form, contributions can be recalculated using the lower income.
